This 15-year-old male is displaying characteristics of borderline personality disorder (BPD) (F60.3). This diagnosis is evidenced by real or imagined feelings of abandonment by his boyfriend, recurrent suicidal behavior and self-mutilating behavior, unstable interpersonal relationships, affective instability caused by a reactivity of mood, and identity disturbance (American Psychiatric Association, 2013). This individual has self-mutilating behaviors as well as previous suicide attempts. The patient has an unstable relationship with his mother, her giving everything she can but not being home much, patient is disrespectful of her. Patient has been displaying a depressed mood, has low energy and a poor self-esteem.
The therapeutic approach I would choose to use with this patient is group therapy using CBT. CBT has been proven to help people suffering with BPD by changing their negative thoughts and feelings about themselves (Association for Behavioral and Cognitive Therapies, 2020). CBT has been shown to be very effective in treating BPD so I have strong hopes for this individual. BPD may take weeks, even years to treat and the individual may need to continue therapy longer than most individuals (National Institute of Mental Health, 2017).
There are a few things that come to mind with legal/ethical considerations when I think about counseling minors. Being sure I have informed consent, types of confidentiality and needs and does not need to be reported as well as what I can and cannot share with the parents. Sometimes it can be hard to know what needs to be shared and what we are unable to share when it comes to working with a minor.
