Analysis and interpretation of Community Assessment Data

Analysis of the primary data gathered revealed that most admissions to the inpatient facilities within the health facilities in the Kissimmee Community related to issues on Osteoarthritis, chest pain, rehab procedures, single live born, trauma related to delivery, pelvic organs disorder, labor related issues, cellutis, bronchitis and urethra infections (Florida Hospital, Kissimmee, 2013).
Analysis of secondary data on the information related to healthy measure of the Kissimmee community was facilitated by the use of online Health Community Network (HCN) tool. This tools offers access to high-quality data and decision support. The tool was essential in determining the levels mortality, morbidity, access to care, risk factors, access to health screenings, and other conditions. The information was benchmarked as per Healthy people 2020 targets.

From the data acquired on hospital utilization, incidence, prevalence and mortality rate, the major issues that need to be addresses are related to heart disease and cancer. These two chronic issues are known to be associated with obesity. Therefore, coming up with obesity intervention will be relevant to eradicate or reduce the rate of chronic diseases. Management practice on chronic disease including the advocating for exercise, nutrition, and enhanced personal accountability will be relevant. There will be need to establish to establish dedicated service lines dealing with sexually transmitted infections, substance abuse, mental conditions, and maternal and child issues.

It has been established that the cardiovascular related problems is one of the critical health issue among the Kissimmee community that needs to be addressed. This plan sets out a framework which can be used to improve on this health issue and ensure general improvement of the quality of life for the population in Kissimmee. The goals of this plan matches with Healthy People 2020 goals that seeks to improve the cardiovascular health and the quality of life through prevention, detection, and treating the risk factors associated with heart attack and stroke (ODPHP, 2017). Raising such awareness will be important in reducing the health disparities that exist within this community based on gender, age, race and socioeconomic factors.
Implementing the plan
For the plan to be successful, some conditions need to be put in place to ensure that the fundamental objectives will be attained. It will be highly relevant to put in place effective channels of communication. The public health community will establish means of communicating effectively the urgency and promise of managing and preventing heart conditions. The successful implementation of the plan will require strategic leadership, partnerships and organization to be put in place. The success of a healthy strategy on preventing heart disease depends on the ability for the plan to spearhead by visionary leaders who will be in a position to maintain strong partnership and collaborations with key stakeholders.
The plan will be requiring being into action by adopting interventions that deal with policy and environmental change with a population-wide capability. The plan will be put in action in way that it helps the goals stipulated on Healthy People 2020. The plan will adequately address all risk factors associated with heart disease.
Justification of the plan
Heart disease has been noted as one of the leading causes of deaths not only on Kissimmee Community but in the whole of United State. The health issues related to heart related problems are known to be costly to the health care system of the nation. However, most the factors leading to heart related diseases are preventable as they relate to high blood pressure, high cholesterol, cigarette smoking, diabetes, lack of physical activity, overweight and obesity. It is therefore important to carry out this plan to help increase the awareness level among the members of the population.
The scope of the plan
The plan will be affected in Kissimmee City in Osceola County in Florida.
