Diabetes prevention and control is the nation’s top public health priority. Diabetes
patients and their guardians are the target population for this study. The personal health
assessment aims to give them a quick, simple, confidential evaluation of their current health and
risks and provide a personalized health plan. However, we are targeting Virginians in the
Hampton Roads area. A survey was conducted of diabetes patients as part of a strategic planning
process to obtain their perceptions of the types of preventive and clinical services available to
them.
A questionnaire can be a very convenient way of quickly collecting information from a
selected target audience. For the results to be meaningful and generalizable, the questionnaire
design must ensure that accurate data is collected. By conducting surveys, researchers can assess
how representative the views and experiences of individuals are. Making important decisions
about people’s opinions and behavior can be accomplished with the help of surveys.
Population trends, health outcomes, and healthcare costs make diabetes a priority for
population health improvement in the United States; In a report released on March 22, 2018, the
American Diabetes Association (ADA) estimates that the costs associated with diagnosed
diabetes had increased from $245 billion in 2012 to $327 billion in 2017 (The Cost of Diabetes |
ADA, n.d.). People with diabetes have more than twice as much medical spending as those
without diabetes (The Cost of Diabetes | ADA, n.d.). Various demographic factors were analyzed
to examine the level of patients with different attributes of diabetes obtaining medical services.

The data collected was qualitative primary data, meaning it was collected by the group, for the
purpose of this community health survey. Qualitative data is “a verbal or narrative form” and
“information obtained via key informant interviews, open-ended survey questions, or focus
groups” (Caron, 2017). Primary data simply means, “collected by investigators for their own
specific purpose” (Caron, 2017). Due to the nature of the situation, this survey was conducted
online, by sending out survey links to participants. By conducting the survey online, it allowed
the group to reach a larger population in a short amount of time. Although this may not be the
best way to obtain the most accurate results, it can be duplicated and modified to meet the needs
of any community and adjusted to change the population.

The survey conducted in Hampton Roads for those age 45 or older, diagnosed with diabetes,
showed that of 100 participants, 15% reside in Virginia Beach, 30% reside in Chesapeake, and
55% reside in Norfolk. 13% of the participants were between the ages 45-49, 28% were 50-59
years old, 29% were 60-69 years old, and 30% were between the ages 70-79. There were 12%
that are American Indian or Alaska Native, 38% are White, 38% are Black or African American
and 12% are More than 1 race. Out of 100 participants, 63 were male and 37 were female, all
participants main language spoken at home is English. Participants were asked if they live alone
or with other family members, in which 10 responded they live alone, 30 said they lived with 1
other person, 10 said they live with 2 other people, 30 said they live with 3 other people, 20 said
they live with 4 other people, and none responded that they live with 6 or more. 40 participants
said that they are adult caregivers, while 60 responded that they are not. Participants were asked

about their highest education level completed, in which 30% responded that they completed
some college, but have no degree, 10% have completed a 2-Year degree, 40% completed a 4Year degree and 20% have completed Graduate level. The populations income shows that 20%
have a household income of $10,000-$24,999, 20% have a household income of $25,000$49,999, 20% have a household income of $50,000-$74,999, 20% have a household income of
$75,000-$99,999, 10% have a household income of $125,000-$149,999, and 10% have a
household income of $150,000-$174,999.

Diabetes is a life-threatening disease that must be continually monitored and effectively
controlled by proper medication and adopting a healthy lifestyle; healthy lifestyles, regular
checkups, and proper medication are all essential for a long, healthy life. Diabetes increases the
risk of early death, and complications related to diabetes can lower the quality of life.
Individuals, healthcare systems, and nations all suffer from high costs associated with diabetes.
As population care for diabetes continues to evolve, future initiatives should focus on tailoring
population care to meet individual patient needs while leveraging advances in clinical
information systems and care integration to manage and prevent diabetes in the future optimally.
