Using the Community Tool Box Chapter 2: Section 1. Carefully read how to develop a Logic Model and how Theory is used to back up your belief about the change that you are proposing for your community. Upload the logic model for this task as one document. The logic model will likely change as you proceed throughout this course and you learn more about measurable outcomes and how to make measurable change.

The second document you will upload is a written paper that discusses your community’s strengths, weaknesses, and problems which the Logic Model shows as the change effort, better known as the program outcome. In a designated section of this paper, you will write about theory and how your self-selected theory supports your conclusion about why this problem exists and how it came into existence.
